Spousal/Partner verification ensures our benefit plan is administered fairly and consistently. As part of our ongoing eligibility review, we are verifying that all enrolled spouses meet the plan’s eligibility requirements.
This page provides a convenient way for you to complete the verification process and securely upload the required documentation. We understand gathering documentation may feel inconvenient; however, this step is necessary to maintain compliance with plan rules and federal regulations.
If you are covering a spouse or partner on our healthcare plan, you must complete the verification process before coverage begins. A $300/month surcharge will be added to your health insurance premium until the Spousal/Partner verification is confirmed.

Instructions
As outlined during enrollment, employees who elect coverage for a spouse or domestic partner who has access to other available health insurance are subject to a $300 per month premium surcharge. Other available coverage includes, but is not limited to:
